3. HIPAA and Protected Health Information
LūM is a HIPAA-covered healthcare provider. Your Protected Health Information (PHI) is handled in accordance with our Notice of Privacy Practices, which is provided to all patients and available upon request at our office.
With your authorization, we may release medical information to:
- Insurance companies for claims processing
- Other providers rendering medical or dental care
- Laboratories fabricating appliances or performing diagnostic work necessary for your treatment

5. Sharing of Information
We do not sell your personal or health information. We may share information with:
- Insurance companies and clearinghouses, for claims processing
- Business associates who perform services on our behalf (e.g., billing, IT, dental labs) under HIPAA-compliant agreements
- Third-party collection agencies, in the event of a delinquent account

- Government agencies or regulators, when required by law
If your account is referred to a third party for collection, this may be reported to credit reporting agencies and reflected in your personal credit history.
