Klauer, DDS and the LūM team, including through in-office and telemedicine visits.

3. HIPAA and Protected Health Information

LūM is a HIPAA-covered healthcare provider. Your Protected Health Information (PHI) is handled in accordance with our Notice of Privacy Practices, which is provided to all patients and available upon request at our office.

With your authorization, we may release medical information to:

  • Insurance companies for claims processing
  • Other providers rendering medical or dental care
  • Laboratories fabricating appliances or performing diagnostic work necessary for your treatment

4. Telephone, Text, and Email Communications (TCPA)

By providing your phone number or email address, you expressly consent to be contacted by LūM and its agents — by phone (including wireless numbers), text message, email, or automated means — for purposes including servicing your account, appointment reminders, and collection of amounts owed. Message and data rates may apply. You may revoke this consent at any time by contacting our office directly.

5. Sharing of Information

We do not sell your personal or health information. We may share information with:

  • Insurance companies and clearinghouses, for claims processing
  • Business associates who perform services on our behalf (e.g., billing, IT, dental labs) under HIPAA-compliant agreements
  • Third-party collection agencies, in the event of a delinquent account

  • Government agencies or regulators, when required by law
