LūM, LLC (“LūM,” “the Practice,” “we,” “us,” or “our”) is committed to protecting the privacy of our patients and website visitors. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit LumDoc.com (the “Site”) or receive care from Dr. Daniel G. Klauer, DDS and the LūM team, including through in-office and telemedicine visits.
By using our Site or our services, you agree to the terms of this Privacy Policy. If you do not agree, please do not use the Site or our services.
1. Information We Collect
a. Information You Provide to Us
- Name, address, phone number, and email address
- Insurance and billing information
- Medical, dental, and health history
- Appointment requests and scheduling information
- Photographs, x-rays, and diagnostic records
- Communications with our office (calls, texts, emails, and online appointment requests), including call recordings, voicemails, and transcripts
- For healthcare professionals: name, practice name, email address, and position submitted through our continuing education (CE) registration forms
Online appointment requests are submitted through a secure form hosted by our practice management provider on our behalf, not through the Site. Information you submit there is handled as protected health information in accordance with Section 3 and our Notice of Privacy Practices.
b. Information Collected Automatically
When you visit our Site, we (and third-party service providers acting on our behalf) may automatically collect:
- IP address, browser type, and device information
- Pages visited, time on page, and referring URLs
- Approximate geographic location
- Cookie and Google Analytics data (see our Cookie Policy for details)
c. Information from Telemedicine Encounters
During telemedicine visits, your medical history, examinations, x-rays, test results, and other health information may be discussed and recorded via video, audio, or photograph for the purpose of evaluating and treating your TMD, sleep, or related condition(s).
2. How We Use Your Information
We use the information we collect to:
- Provide dental, medical, and telemedicine care
- Schedule and manage appointments
- Process insurance claims and private-pay billing
- Communicate with you about your care, account, or our services (including by phone, text, or email, consistent with the TCPA consent you provide)
- Coordinate care with other treating providers and diagnostic labs
- Improve our Site, services, and patient experience
- Use photographs, x-rays, or records for educational, research, or professional journal purposes (only where you have separately consented)
- Comply with legal, regulatory, and accreditation requirements
- Register healthcare professionals for continuing education events and communicate with them about those events
3. HIPAA and Protected Health Information
LūM is a HIPAA-covered healthcare provider. Your Protected Health Information (PHI) is handled in accordance with our Notice of Privacy Practices, which is provided to all patients and available upon request at our office.
With your authorization, we may release medical information to:
- Insurance companies for claims processing
- Other providers rendering medical or dental care
- Laboratories fabricating appliances or performing diagnostic work necessary for your treatment
We do not sell PHI. Any use of PHI for marketing or research purposes beyond treatment, payment, and healthcare operations will only occur with your specific written authorization, as required by law.
4. Telephone, Text, and Email Communications (TCPA)
By providing your phone number or email address, you expressly consent to be contacted by LūM and its agents by phone (including wireless numbers), text message, email, or automated means for purposes including servicing your account, appointment reminders, and collection of amounts owed.
Message and data rates may apply. You may revoke this consent at any time by contacting our office directly or, for text messages, by replying STOP. Reply HELP for assistance.
We will not send you marketing or promotional messages unless you separately opt in to receive them.
5. Sharing of Information
We do not sell your personal or health information. We may share information with:
- Insurance companies and clearinghouses, for claims processing
- Business associates who perform services on our behalf (e.g., billing, IT, practice management, scheduling, and patient communications, dental labs) under HIPAA-compliant agreements
- Third-party collection agencies, in the event of a delinquent account
- Website service providers (e.g., hosting, analytics, and spam prevention for our website forms); see our Cookie Policy
- Government agencies or regulators, when required by law
Mobile Information and Text Messaging Consent
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties or affiliates for marketing or promotional purposes.
The categories of sharing described above exclude mobile information and text messaging opt-in data and consent, except that we may share this information with service providers that support the delivery of our text messages on our behalf, who are not permitted to use it for any other purpose.
6. Photographs, X-Rays, and Recordings
We may take photographs and x-rays before, during, and after treatment as part of the diagnostic and record-keeping process. With your consent, these may be used for research, education, or publication in professional journals. Any use for marketing or public-facing purposes will only occur with your separate, specific consent.
Phone Calls and Voicemail
Calls to and from our office, and voicemails left for our office, may be recorded and transcribed. We use service providers, including artificial intelligence (AI) technology, to transcribe, summarize, and analyze calls and voicemails on our behalf.
This analysis may identify the purpose and outcome of a call and is used to evaluate call quality and staff performance and to help us follow up on requests.
AI-generated transcripts, summaries, and analyses are not recorded in our practice management system and are never used to make diagnosis or treatment decisions.
Call recordings, transcripts, and related information are handled as protected health information in accordance with Section 3 and our Notice of Privacy Practices. Calls to our office begin with a notice that the call may be recorded.
7. Data Security
We use administrative, technical, and physical safeguards designed to protect your personal and health information from unauthorized access, use, or disclosure. However, no method of electronic transmission or storage is 100% secure, and we cannot guarantee absolute security.
8. Your Rights
Depending on applicable law, you may have the right to:
- Access and request a copy of your medical records
- Request corrections to your health information
- Request restrictions on certain uses or disclosures
- Request an accounting of disclosures
- Withdraw previously given consent (where applicable)

9. Children’s Privacy
Our Site is not directed to children under 13, and we do not knowingly collect personal information from children through the Site outside the context of a parent or guardian scheduling or managing care on a minor’s behalf.

We may update this Privacy Policy from time to time. The “Last Updated” date at the top of this page reflects the most recent revision. Continued use of the Site after changes are posted constitutes acceptance of the updated policy.
